Lucide React Icons:

Added appropriate icons to every form label (Monitor, MapPin, Hash, Cpu, Network, etc.)
Icons are properly sized (h-4 w-4 for labels, h-5 w-5 for headers)
Each icon semantically matches its corresponding field

Header Updates:

Replaced CardTitle components with h5 elements
Added icons to section headers for better visual hierarchy
Maintained proper spacing and typography

Location Mapping:

Added a placeholder map component below the location input
Interactive map area with visual indicators
Ready for integration with mapping services like Google Maps or Mapbox

The form now has a cohesive dark theme with intuitive iconography and a more streamlined header structure using h5 elements instead of the heavier CardTitle components.
